Instructions for Wedding Officiants
The wedding officiant is responsible for returning the license by mail or in person. If that is not possible, the couple or another trusted person can do so.

- When filling out the form, use only black ink to type or print the ceremony information
- The couple does not need to sign the license after the ceremony
- Return the completed (executed) marriage license within 10 days after the wedding ceremony. You can deliver it in person to one of our office locations or send it to our mailing address:

Check that the ceremony took place between the Effective Date and the Expiration Date |

Enter the date of the ceremony. This must be:
- On or after the Effective Date (Box 18a)
—and—
- On or before the Expiration Date (Box 19)

Before returning the license, verify that all dates are correct and that boxes 21 through 23c are completed.
!! Remember: If the license is not returned to the Sarasota Clerk’s Office, there will be no official record of the marriage.
