Jurors requesting to be disqualified or excused from jury duty must complete the Juror Affidavit.
Jurors reporting for jury duty must complete the Juror Questionnaire. To expedite the juror check-in process, complete the Juror Questionnaire and bring it with you on the day you report for jury duty.
