Instructions for Submitting Unclaimed Moneys Claim

RE:      Instructions for Submitting an Unclaimed Moneys Claim

Pursuant to Florida Statute Chapter 116.21, a list of unclaimed moneys held by Karen E. Rushing, Sarasota County Clerk of the Circuit Court and County Comptroller, is published in the July 29, 2016 edition of the Gulf Coast Business Review.

Persons having or claiming interest in such funds (payees) shall file their written claim with the Clerk of the Circuit Court and County Comptroller on or before September 1, 2016. After such date, the unclaimed moneys shall be declared forfeited to the Clerk and Comptroller.

To file a written claim, an Affidavit for Stop Payment form must be completed, notarized and submitted to the Clerk of the Circuit Court and County Comptroller, and received by the Clerk’s office on or before September 1, 2016. A written claim may be filed by mail, in person or by phone.

TO FILE VIA INTERNET / MAIL:

  • PRINT the Affidavit for Stop Payment form.
  • CALL the Clerk Finance department at (941) 861-7640 to obtain the following :
    • Check date
    • Check number
    • Amount of original check
    • Description
  • COMPLETE the Affidavit for Stop Payment form with the above information.
  • TAKE the form to a Notary Public.
  • SIGN the form in the presence of the Notary Public.
  • MAIL the completed, signed and notarized form to:

Upon receipt of a fully completed, signed and notarized Affidavit for Stop Payment, the Clerk Finance department will re-issue the referenced check, and will mail it to the address identified on the form.

TO FILE BY TELEPHONE / MAIL:

  • CALL the Clerk Finance department at (941) 861-7640.
  • ADVISE that you claim an interest in the funds advertised in the newspaper. The Clerk Finance department will complete the Affidavit for Stop Payment form, and will mail it to you for your signature.
  • TAKE the form to a Notary Public.
  • SIGN the form in the presence of the Notary Public.
  • MAIL the completed, signed and notarized form to:

TO FILE IN PERSON:

  • VISIT one of the Clerk and Comptroller’s two office locations:
    • Main Sarasota office: Historic Courthouse, 2000 Main Street, Sarasota, FL 34237
    • Venice branch office: R. L. Anderson Center, 4000 S. Tamiami Trail Venice, FL 34293
  • ADVISE that you claim an interest in the funds advertised in the newspaper. A customer service representative will complete the Affidavit for Stop Payment form.
  • SIGN the form in the presence of the customer service representative.
    NOTE: Customer must produce acceptable personal photo identification, such as:
    • Valid, unexpired United States state-issued driver’s license
    • Valid, unexpired United States state-issued identification card
    • Valid, unexpired U.S. military identification
    • Valid, unexpired Passport

Upon receipt of a fully completed, signed and notarized Affidavit for Stop Payment form, the Clerk Finance department will re-issue the referenced check, and will mail it to the address identified on the form.
